When you establish care at Terry Reilly you will be asked to select a healthcare provider, who works as part of larger healthcare team that will be responsible for your health. Your provider (which could be a doctor, physician assistant or nurse practitioner) is a critical part of your healthcare team.

How Patient Centered Care Works for You

At Terry Reilly, Your Health Care Team:

Is available – you can communicate with your care team by phone or through the Patient Portal and you can get an appointment quickly, even on the same day, when needed.
Knows you and remembers your health history - We should know enough about your personal and family situation to suggest treatment options that make sense for you.
Makes sure you understand your condition - and how to take care of yourself. We help you sort through your options and make decisions about your care.
Helps you coordinate your health care – even if we are not providing the care. We will help you find a specialist, get appointments, and make sure specialists have all the information they need. Your health care team will also be sure you know what the specialists say and what it means for you.

How to get the Most from Your Health Care Team

What Your Health Care Team Should Do:
  • Learn About You – we should get to know you, your family, your life situation, and preferences. We will remember these details about you when you seek care, and suggest treatments that make sense to you. We treat you as a full partner in your care.
  • Communicate with You - we will make sure you know and understand all your options for care. We will help you decide what care is best for you. Sometimes more care is not better care. We will ask you for feedback about your experience.
  • Support You in Caring for Yourself – We will make sure you leave the office with a clear idea of how to care for yourself and help you set goals for your care. Then we help you meet your goals one step at a time.

What You Can Do:

  • Learn About Caring for Yourself - Know that you are a full partner in your own care and learn about your condition and what you can do to stay as healthy as possible. Follow the plan that you and our health care team have agreed is important for your health. And, If you have questions, ask!
  • Communicate with your health care team - Always bring a list of questions to each of your appointments. Bring your medication bottles or a list of any medicines, vitamins, or remedies you use. Always tell our team when you don’t understand something. Ask them to explain it in a different way. Always tell your health care team if you get care from other health professionals so they can help coordinate the best care possible.
Who is Part of My Health Care Team?

Your health care team includes your Provider, your Nurse, and other health care professionals, like a pharmacist, a referral coordinator and a case manager. These team members are like having “health coaches” who help you get healthy, stay healthy, and get the care and services that are right for you. Everyone on the care team is trained to ensure your health information is kept confidential.
