Caldwell, Idaho: Terry Reilly Health Services and Moonlake Consulting are pleased to announce the initiation of a joint project that will combine housing and health care to benefit the Caldwell community. Together, they will be constructing a four-story structure in Downtown Caldwell. The first floor will be home to a Terry Reilly clinic, providing primary, dental and behavioral health care. The additional 3 floors will be affordable apartments for individuals or families age 55 or older.
“We are really excited about this partnership, merging housing and healthcare in the heart of Downtown Caldwell,” said Terry Reilly’s CEO, Heidi Hart. “We believe it promotes the health and well-being of Caldwell residents, while supporting the City of Caldwell’s downtown development efforts.”
The first floor of the building will be a Terry Reilly clinic integrating medical, dental, and behavioral health services, utilizing the Patient Centered Medical Home (PCMH) philosophy in its design. The health care site will include exam rooms, lab, x-ray, dental operatories, counseling offices, group rooms, and more. The residential portion of the structure will include 50-units of housing in three stories above the clinic.
“We have been searching Caldwell for the right opportunity for several years and are excited to partner with Terry Reilly Health Services in the project. This collaboration has the possibility to continue the visionary transformation of the downtown Caldwell core currently led by the City,” says Chance Hobbs, Manager of Moonlake Consulting.
This shared project maximizes resource efficiency and land utilization, creating access to both housing and affordable medical, dental, mental health and case management services for a shared senior population, as well as the broader Caldwell community. For those who live in the complex, they will find an attractive rental community in Downtown Caldwell that allows for immediate service availability and access to Downtown Caldwell, eliminating transportation barriers facing seniors with compounding quality of life implications.

Canyon County currently has 1,600 units of affordable housing but availability is at 0%. The Southwestern Idaho Cooperative Housing Authority had more than 1,400 families on its waiting list in the Southwestern Idaho region. Experts report the city would need to build at least 1,000 housing units to address the current shortage. Cleveland Square will provide relieve to the community housing shortage.
Terry Reilly began providing health care in Caldwell in 2006 with the introduction of the clinic at 2005 Arlington Avenue, providing medical and behavioral health services to members of the community. Dr. Bob Ring and Dr. Richard Roberge made the Caldwell Arlington clinic possible when they generously donated the building that year. The clinic has grown to serve approximately 3,636 patients in 2017, 49% of which were uninsured. The new clinic will provide for increased access to health care in the Caldwell community and will allow Terry Reilly to bring affordable dental care to Caldwell.
Terry Reilly Health Services purchased the Pennywise building and property in 2017, with plans to bring a clinic to downtown Caldwell. With the announcement of this joint venture, Terry Reilly is initiating a $1 million capital campaign to raise the funds necessary to remodel their portion of the project. The housing portion of the project is made possible due to an Affordable Housing Tax Credit Award from Idaho Fair Housing and Finance.
Groundbreaking for the Cleveland Square is planned for February 2019, with an opening in June 2020.
